   Program Coordinator-Residential POSITION STATUS: 40 Hours/Week, 52 Weeks/Year QUALIFICATIONS: Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field.† Minimum of two (2) yearsí work experience with the developmentally disabled including one (1) year in a supervisory capacity OR enrollment and successful completion of the Agency Supervisory Training program.† Must meet the Office of People With Developmental Disabilities (OPWDD) regulations regarding Qualified Intellectual Disabilities Professional (QIDP). Valid NYS driverís license acceptable to Agency insurance standards. GENERAL RESPONSIBILITIES: Directly responsible to the Director and Assistant Director of Residential Services with respect to agency and program policies, practices and procedures. Responsible for the development and monitoring of the Residential Habilitation Plan, Behavior Plan and Protective Oversight Plan for all assigned individuals. This includes the training, supervision and evaluation of residential staff in the implementation of support plans through periodic reviews. Inclusive in this responsibility is the coordination of all residential services directly related to the Individualís Support Plan and having a working knowledge of State and Federal regulations. SPECIFIC RESPONSIBILITIES: 1. Oversees the development, training and supervision of all residential staff by the following methods: a. Participates in the development of: reviewing and updating the personís Individual Support Plan in concert with the 3Iís and P and person centered philosophy. Coordinates the development of; reviewing, updating and signing the personís Protective Oversight Plan, Res Hab Plan, DDPII or other relevant service plans. (Comprehensive Functional Assessments for ICFís) b. Trains staff in the implementation and data collection of all individual support plans and services. c. Evaluates through informal observation and periodic evaluation of staffs' performance implementing all residential support programs, including active treatment, choice and independence and the personís progress in achieving their valued outcomes. d. Evaluates the personís satisfaction with activities, interventions and/or therapies. Ensures program integration through periodic observations of Individualís Day Program. 2. Participates as a member of the interdisciplinary team through: a. Participates in and attends individual oriented meetings, to include admission/discharge, 30-day reviews, annual and semi-annual meetings. b. Reviews and recommends with the person and their team the revisions of outcomes, or selection of different outcomes/activities, interventions or therapies and specify the appropriate action to be taken and time-frame for achievement of the revised/changed outcome. c. Identifies noteworthy observations relevant to the personís receipt of services. d. Ensures all routines and specialized services are delivered in a timely and appropriate fashion according to the individualís ISP and/or CFA.. e. Coordinates services with outside agencies as appropriate. 3. Ensures the documentation of: a. Appropriate data collection for all Residential Habilitation, Behavioral and/or other individual program plans. b. A monthly review of the personís progress is completed for all support plans and services c. Completes 90-day, semi-annual and annual summaries or assessments of the personís progress relative to his/her receipt of residential services in a timely manner. d. Signature for informed consent. 4. Participates in in-service training on a regular basis through: a. Attends all mandatory in- services and training. b. Instructs in-services specific to goal planning, observation and recording, Activities in Daily Living, 3I and P, and the Five Governing Principles. c. Performs behavior management (SCIP) when necessary. 5. Performs all job duties in a safe manner by observing all Agency safety rules, procedures and practices. 6. Enrolls and withdraws individuals in OPWDD Service delivery system by accessing CHOICES electronic record keeping/sharing system and updates records as needed. 7. Performs all other duties as assigned by the Director and/or Assistant Director of Residential.
