   QUALIFICATIONS: Bachelor of Science degree in Human Services, Education, Special Education, Psychology or a related field.  Minimum of two (2) years’ work experience with the developmentally disabled including one (1) year in a supervisory capacity OR enrollment and successful completion of the Agency Supervisory Training program.  Must meet the Office of People With Developmental Disabilities (OPWDD) regulations regarding Qualified Intellectual Disabilities Professional (QIDP). Valid NYS driver’s license acceptable to Agency insurance standards. GENERAL RESPONSIBILITIES: Responsible for the development, training and supervision of all programs for all assigned individuals. This includes the training, supervision and evaluation of staff in the area of implementation of programs and the ensurance of quality of programming through periodic review. Inclusive in this responsibility is coordination of all individual services directly related to their Individual Service Plan/CFA. Directly responsible to the Habilitation Coordinator with respect to questions regarding policies, practices and procedures. Supervised by the Habilitation Coordinator. SPECIFIC RESPONSIBILITIES: 1.Develops and/or reviews Individual Service Plan (ISP) and/or Comprehensive Functional Assessment (ISP/CFA) and ensures it corresponds to the day services and residential plans, (ensures annual, physical, TB test, behavior plans, personnel protection training, self medication test, diets, medications, allergies, and safeguards are included) 2.Oversees the development, training, supervision and evaluation of all classroom staff. 3.Ensures all clinical services are developed and implemented by the IDT. 4.Assists staff in the development of person centered outcomes which correspond with the individual ISP/CFA. 5.Performs individual Quality Assurance checks in programs, ensuring that active treatment, active learning and quality services are being provided. 6.Attends and participates in individual ISP/CFA meetings. 7.Assists with providing coverage for daily staffing patterns in the programs, routinely and or in the absence of direct care staff. 8.Assists in locating potential worksites for individuals who want employment opportunities and placement. 9.Oversees the writing of all non-clinical programs. 10. Evaluates through informal observation and formal periodic evaluation of staff's performance implementing programs, developing person-centered strategies, and developing activities which correspond with the individual’s ISP/CFA. 11. Acts as a liaison with outside providers. 12. Oversees the development and accuracy of all program plans and ensures appropriate data collection for all program plans and clinical support. 13. Ensures that monthly review of progress in all programs is completed in a timely manner. 14. Ensures that programs are revised or changed as needed. 15. Participates in in-service training on a regular basis as required. 16. Performs in-services specific to goal planning, observation, recording and data collection.
