   PROGRAM: Care Management JOB TITLE: Care Manager QUALIFICATIONS: • A Bachelor’s degree in Human Services or a CASAC; at least 2 years’ experience providing direct services for individuals with mental illness, developmental disabilities, or substance use disorders, or linking those individuals to essential services • Clearance with all relevant background checks as per TSA policies and governing regulations • A vehicle, valid NY State Driver's License and a driving record acceptable to the TSA automotive program are required • Proficient in MS Office Suite and able to learn new data platforms PREFERRED QUALIFICATIONS: • Familiarity with Trauma Informed Care • Experience with Health Home Care Management and/or the Criminal Justice System • Familiarity with New York State Department of Health (DOH) Health Home Care Management (HHCM) regulations and guidelines PERSONAL REQUIREMENTS: • Be understanding of the needs of sensitive populations and demonstrate empathy • Have well-developed professional boundaries and high ethical standards • Demonstrate excellent organizational, time management, and communication skills • Possess strong crisis management and problem-solving skills • Have an ability to function as a member of team member and work independently SCOPE: Providing DOH Health Home Care Management services to adult clients. The Care Manager reports to the Program Director and Supervisor. JOB DESCRIPTION: The Care Manager will have a broad range of duties including, but not limited to, the following: • Provide Health Home Care Management services per DOH requirements • Conduct comprehensive assessments and develop person-centered strength based plans of care • Provide liaison services to hospitals, primary care doctors, outpatient mental health providers, and community support services for medical, behavioral, social and housing needs • Participate in the referral process to Home and Community Based Service Providers (HCBS) in collaboration with MCOs • Advocate with appropriate agencies for services to assist members with scheduling and coordinating services • Monitor, evaluate and document client progress with respect to care plan goals in a computer database • Meet accurate and timely reporting, documentation, and record requirements pursuant to Health Home standards • Comply with confidentiality laws governed by the Health Home consent form, NYS and HIPAA • Provide crisis intervention and crisis management • Provide supportive counseling and skill training to each client • Maintain up to date knowledge of all rules, guidelines, policies and procedures of our Health Home (Care Central) and all regulatory oversight agencies • Carry on-call responsibility as assigned • Additional responsibilities as assigned by the Program Director TSA is a Human Services Agency dedicated to improving lives.
