   PROGRAM: Care Management Crisis Team JOB TITLE: Peer Advocate MINIMUM QUALIFICATIONS: High School Diploma or GED Strong understanding of issues related to living with a mental health or substance use disorder, physical and/or cognitive challenges, and/or HIV/AIDS A willingness to become a Certified Peer Specialist Clearance with all relevant background checks as per TSA policies and governing regulations A vehicle, valid NY State Driver's License and a driving record acceptable to the TSA automotive program are required Proficient in MS Office Suite and able to learn new data platforms PREFERRED QUALIFICATIONS: Familiarity with Trauma Informed Care Experience with Health Home Care Management and/or the Criminal Justice System Familiarity with New York State Department of Health (DOH) Health Home Care Management (HHCM) regulations and guidelines Experience providing outreach and/or strong advocacy for individuals in need of social supports Is a Certified Peer Specialist, a Certified Recovery Peer Advocate, or has a comparable certification PERSONAL REQUIREMENTS: Be understanding of the needs of sensitive populations and demonstrate empathy Have well-developed professional boundaries and high ethical standards Demonstrate excellent organizational, time management, and communication skills Possess strong crisis management and problem-solving skills Have an ability to function as a member of team member and work independently JOB DESCRIPTION: The Peer Advocate will have a broad range of duties including, but not limited to, the following: Identify individuals who may be eligible for Health Home Care Management services and make referrals Work with the Embedded Care Manager to identify individuals who may be in need of follow-up support and ensure that those services are provided Engage with individuals who need emotional support and assist individuals to communicate their needs to other service providers Act as an advocate for individuals in crisis; remain with them during stressful interactions to provide emotional support and model effective coping skills Provide supportive counseling for clients as necessary Accompany individuals to meetings and appointments as necessary Advocate for individuals in the process of obtaining supportive services Provide support with basic needs to individuals in the Care Management program (transportation, shopping, etc.) Provide liaison services to hospitals, primary care doctors, outpatient mental health providers, and community support services for medical, behavioral, social and housing needs as appropriate Be available to care management clients as a resource; act as a liaison with their primary Care Manager to coordinate follow-up Outreach individuals in the community Meet accurate and timely reporting requirements pursuant to regulatory standards Comply with confidentiality laws governed by the Health Home consent form, NYS and HIPAA Maintain up to date knowledge of all rules, guidelines, policies and procedures of all applicable Health Homes and all regulatory oversight agencies Attend mandatory agency trainings Additional responsibilities as assigned TSA offers a generous benefit package which includes: Vacation, Sick, Personal Times, Life, Health, Vision Insurances, 403(b) and more!
