   PROGRAM: Care Management JOB TITLE: Youth Care Manager QUALIFICATIONS: • A Bachelor’s degree in Human Services or a related field and at least two years’ experience working with SED/at risk youth • Clearance with all relevant background checks as per TSA policies and governing regulations • A vehicle, valid NY State Driver's License and a driving record acceptable to the TSA automotive program are required • Proficient in MS Office Suite and able to learn new data platforms PREFERRED QUALIFICATIONS: • Familiarity with Trauma Informed Care • Experience working in a Health Home Care Management setting • Familiarity with New York State Office of Department of Health (DOH) Health Home Care Management (HHCM) regulations and guidelines • Knowledge of the NYS Child and Adolescent Needs and Strength Assessment (CANS) PERSONAL REQUIREMENTS: • Be understanding of the needs of sensitive populations and demonstrate empathy • Have well-developed professional boundaries and high ethical standards • Demonstrate excellent organizational, time management, and communication skills • Possess strong crisis management and problem-solving skills • Have an ability to function as a member of team member and work independently SCOPE: Providing DOH Health Home Care Management services to youth clients. The Youth Care Manager reports to the Program Director and Program Supervisor. JOB DESCRIPTION: The Youth Care Manager will have a broad range of duties including, but not limited to, the following: • Providing Health Home Care Management services per DOH requirements while meeting minimum caseload requirements • Meeting accurate and timely reporting, documentation, and record requirements pursuant to Health Home standards • Complying with confidentiality laws governed by the Health Home consent form, NYS and HIPAA • Assessing client eligibility and level of interest and the family’s needs and strengths; completing the CANS-NY, safety and crisis plan, emergency planning assessment, etc. • Planning for client’s needs (e.g. treatment planning, discharge planning, transitional and aftercare services) and linking client to needed services (e.g., mental health, school, health, legal, recreational/ social, etc.) • Monitoring client's progress (mental status, medication compliance, etc.) and timely documentation of information • Providing crisis management (in home and other community settings) • Providing supportive counseling/skill building/planning and implementing social/recreational activities for all clients • Reviewing and updating treatment plans/plans of care • Advocating for clients, siblings and family members when necessary • Assisting service systems and providers in developing needed program/services for clients • Purchasing services/items for clients per case management plan or in an emergency • Providing on-call coverage as assigned • Additional responsibilities as assigned by the Program Director TSA offers a generous benefit package which includes: Health, Dental, Vision, Life Insurance(s), 403(b), Vacation, Sick, Personal Times and your Birthday Off
