   PROGRAM: Care Management JOB TITLE: AOT Care Manager QUALIFICATIONS: • A Bachelor’s degree in Human Services or a CASAC; at least 2 years’ experience providing direct services for individuals with mental illness, developmental disabilities, or substance use disorders, or linking those individuals to essential services • Clearance with all relevant background checks as per TSA policies and governing regulations • A vehicle, valid NY State Driver's License and a driving record acceptable to the TSA automotive program are required • Proficient in MS Office Suite and able to learn new data platforms PREFERRED QUALIFICATIONS: • MSW, or other Master’s level clinical degree • Familiarity with Trauma Informed Care • Experience working in a Health Home Care Management setting • Familiarity with New York State Department of Health (DOH) Health Home Care Management (HHCM) regulations and guidelines, including Health Home Plus • Familiarity with Kendra’s Law; Assisted Outpatient Treatment (AOT) • Knowledge of the NYS Health and Recovery Plan (HARP) PERSONAL REQUIREMENTS: • Be understanding of the needs of sensitive populations and demonstrate empathy • Have well-developed professional boundaries and high ethical standards • Demonstrate excellent organizational, time management, and communication skills • Possess strong crisis management and problem-solving skills • Have an ability to function as a member of team member and work independently SCOPE: The AOT Care Manager will have primary responsibility for providing NYS Care Management services to AOT recipients and other program participants that qualify for Health Home Plus Care Management. The AOT Care Manager reports to the Program Director and Program Supervisor. JOB DESCRIPTION: The AOT Care Manager will have a broad range of duties including, but not limited to, the following: • Provide Health Home Plus Care Management services per DOH requirements to primary caseload of clients; assist other Care Managers as needed • Provide liaison services to hospitals, primary care doctors, outpatient mental health providers, and community support services for medical, behavioral, social and housing needs • Advocate with appropriate agencies for services to assist members with scheduling and coordinating services • Conduct comprehensive assessments and develop person-centered strength based plans of care • Monitor, evaluate and document client progress with respect to care plan goals and progress notes in a computer database • Meet accurate and timely reporting, documentation, and record requirements pursuant to Health Home standards • Comply with confidentiality laws governed by the Health Home consent form, NYS and HIPAA • Provide crisis intervention and crisis management • Provide supportive counseling and skill training to each client • Conduct HARP assessments and a create HARP person-centered plans of care in coordination with a client’s supports, providers, and managed care organizations (MCOs) • Maintain up to date knowledge of all rules, guidelines, policies and procedures of our Health Home (AHI) and all regulatory oversight agencies • Carry on-call responsibility as assigned • Additional responsibilities as assigned by the Program Director TSA offers a generous benefit package which includes: health, dental, vision, life insurance(s), 403b, vacation, sick and personal times........even your birthday off!
